Release Form for Summer 2021
Each camper must have signed and submitted the AMC Release form prior to arriving at camp. Parents or guardians must sign a release for each child under the age of 18. Each copy of the form has signature space for two adults and two minors. Use additional copies if needed.

Signed release forms can be submitted in several ways:
- Print and sign a copy of the Release form. Photograph the signed copy of each release form. Verify that the entire document appears in the photo and
that the text and signatures are readable. (Click here for an example). Email the photograph to the Registrar: elcRegistrar@outdoors.org
with "Week # Release Form"* as the subject.
- Print and sign a copy of the release form. Scan the signed copy of the each release form into a PDF file.
Verity that the entire document appears in the PDF and that the
entire text and signatures are readable. Email the PDF to the Registrar:

"Week # Release Form"* as the subject.
- Using Adobe Reader, electronically complete and sign the release form. Save the signed copy, Email the saved copy of the signed PDF to the
Registrar: elcRegistrar@outdoors.org with the
"Week # Release Form"* as the subject. (This option requires a copy of Adobe Reader and that the user is
familiar with the Fill & Sign features of that software.)
- Print and sign a copy of the Release form. Mail the signed form to the elcRegistrar at:
